Important Information for New Patients

Health History Information

We will send you a comprehensive health history form before your first appointment. The history that you submit will provide vital information that will aid us in formulating a diagnosis and a treatment plan for your particular needs. The reason that we send this form to you is so that you can fill it out as accurately and in as much detail as possible in the comfort of your own home without any time constraints. Other insurance forms and registration forms will be included.

Before you make your initial appointment

Contact your medical and or dental insurance provider to see if these TMD and Orofacial Pain services are a covered benefit under your insurance plan. If you have any questions about the referral process, please call us at 734-998-6320.

PCP Referral (Primary Care Provider)

  • Specifically ask if you need a PCP referral. Many insurance companies require a PCP referral to determine eligibility for payment.

    • Note: you may also want to ask about any co-pays, deductibles, and restrictions or exclusions in your policy for this service(s).

    • You must ask if your treatment will be covered if it is treated by Dr. Lawrence Ashman, a general dentist whose practice is limited to TMD, Orofacial Pain and Sleep Apnea.

      • Note: A referral does not guarantee coverage/payment.

Consultation Referral

Your healthcare provider should provide you with a Consultation Referral. This may or may not be your PCP. The Consultation Referral is different from the PCP referral. It may be from a physician, dentist, physical therapist, etc. that has sent you to our clinic for your particular condition and with whom we will correspond regarding your treatment.

Payment Information

Payment for each visit is expected at the time of service. This includes all co-pays, percentages and deductibles.

Note: We do not accept some insurance types at our clinic.

Payment for services can be made by cash, check, money order, or credit card. Our clinic accepts most major credit cards.

Cancellation Policy:

Our office requires a one full business day notice for any cancellation.
